ADMISSION REQUIREMENTS

Applicants seeking admission into the program must:

  • Must be at least 16 years of age or older

  • Complete application form

  • Completed comprehensive Background check with fingerprinting

  • Submit a recent Physical examination

  • Provide up to date PPD/TB testing documentation

  • Submit proof of the following immunizations and vaccinations:

    -influenza (flu shot)
    -COVID-19 Vaccination
    -Hepatitis B
    -MMR (Measles, Mumps, and Rubella)
    -Varicella
    -DPT Vaccination
